Oxford Study to Deliberately Infect Volunteers with Malaria to Combat Relapsing Infections

A groundbreaking new study led by the Oxford Vaccine Group aims to better understand the complexities of malaria, specifically Plasmodium vivax, a type of malaria that has the ability to remain dormant in the liver for months or even years before reactivating. This innovative research is set to provide vital insights into how to prevent the relapses that affect over 80% of global malaria cases.

The Importance of Understanding Malaria Relapses

Malaria is caused by a parasite spread through mosquito bites, and its most common form, Plasmodium vivax, is known for its ability to remain dormant within the liver. This feature causes recurring infections, which can be difficult to control and treat. In fact, relapsing infections account for the majority of malaria cases worldwide, and current treatments have not been able to address the parasite’s dormant phase.

To gain a deeper understanding of this phenomenon, researchers at Oxford are embarking on a world-first study, aiming to explore how the immune system responds to malaria relapses and to develop methods to prevent them altogether. The study, which is being conducted in collaboration with Radboud University Medical Centre in the Netherlands, will deliberately infect healthy volunteers with the malaria parasite to observe the progression and treatment of relapsing infections under controlled conditions.

A Controlled Experiment with Human Volunteers

The study will involve five healthy volunteers aged 18 to 45, who will travel to the Netherlands to be bitten by mosquitoes carrying the Plasmodium vivax parasite. These participants will be carefully monitored throughout the study as researchers aim to recreate the conditions that lead to malaria relapses.

Once the parasite is detected in the volunteers’ blood, they will be immediately given anti-malarial medication to combat the active infection. However, this medication will not target the dormant parasites in their liver, which is the focus of the research. For the next six months, participants will receive regular check-ups and 24/7 medical support to ensure their safety while studying any potential relapses.

After the monitoring period, all participants will receive a final round of treatment to eradicate the dormant parasites in the liver. Following the study, they will be closely followed for five additional years to track any unexpected relapses.

Why Is This Research So Important?

The primary goal of this study is to develop more effective treatments for Plasmodium vivax malaria, particularly for preventing the relapsing infections that continue to cause widespread issues in malaria-endemic regions. If successful, the study could serve as a valuable model for testing new vaccines or drugs designed to eliminate the parasite from the liver and stop relapses entirely.

Professor Angela Minassian, the chief investigator for the trial, explained the significance of the study: “This proof-of-concept study is the first of its kind to safely introduce relapsing vivax malaria infections in healthy volunteers under controlled research conditions. By doing so, we aim to answer key questions about relapses and how the immune system responds to them.”

If successful, the findings from this research could have a major impact on how the global medical community combats malaria, potentially transforming the way treatments and vaccines are developed. The long-term aim is not only to reduce malaria infections worldwide but also to eliminate relapsing malaria altogether, saving countless lives in the process.
